The Insurance Division of the Office of Legal Affairs administers the University Self Insurance Program (USIP) that provides medical malpractice protection for its hospitals and employed clinical staff of The Ohio State University Wexner Medical Center.
Request for Claims History/Insurance Coverage Verification
Requests for an insured's claims history or insurance coverage verification may be directed to the insurance general email box Insurance.USIP@osumc.edu.
If a request is submitted by a third party, the request must include a signed release by the clinician.
Once all required information is received, the standard turnaround time is five business days.
